Answers to Frequently Asked Questions

1. Do I need a referral to see Dr. Bernabe?
     --If you have PPO insurance or Medicare, YOU DON'T NEED A REFERRAL ! You can call our office directly to set up
        an appointment. If you have HMO insurance, then you need a referral from your primary care physician. You
        can also be referred by the emergency room physician if you were seen at the hospital emergency department.
        MOST OF OUR NEW PATIENTS ARE SELF-REFERRED AND REFERRED BY FAMILY/FRIENDS OF PREVIOUS
        PATIENTS THROUGH WORD-OF-MOUTH. Our reputation in Kings County is unmatched. Simply speaking,
        we are the best!

2. What type of insurance do you accept?
     --We accept all private and commercial insurance in our private office, located inside the Lacey Medical Plaza.

3. Do you accept Sante insurance or MediCal?
     --Please call our office for information.

4. Do you handle emergencies and walk-ins?
     --If you have an emergency, please call 9-1-1 or proceed to the nearest emergency department. We accept     
        walk-ins and same day appointments on a case-by-case basis. Call our office for availability.

5. Do you treat back pain, back problems, neck pain, lumbar/cervical/spine problems?
      --NO.

6. What do I need to bring for my appointment?
      --If you are a new patient, plan to arrive at least 15 minutes prior to your scheduled appointment to complete
         a medical history questionnaire form and to review our Privacy Policy form. Please bring your insurance
         card, identification card, and referral form (if applicable). If you have X-rays, please bring them with you.
         If an MRI was performed, please bring the radiologist's report. If you forgot to bring your X-rays or if you
         have none, Dr. Bernabe will take the necessary X-rays in the office using digital imaging.

7. What is your policy on co-pays?
      --Co-pays are expected to be paid in full at time of service. We accept cash, check, credit, and debit cards.
         Please review your insurance card to determine your co-pay amount.

8. What is your policy on disability forms?
       --Due to the enormous amount of paperwork involved with disability forms, please allow us up to 1 week to
          complete them. We charge a nominal fee. We DO NOT carry blank disability forms. They need to be supplied
          by your employer or you can pick one up at the EDD office located on Irwin St. Please bear in mind that this  
          service is provided to our patients as a courtesy, not an obligation.

9. What is your policy on medication refills?
       --We do not refill medication requests after hours or during weekends. Please make sure your request is
           is in by noon on Friday. No requests will be honored after this time.

10. Does Dr. Bernabe provide second opinion consultations?
        --Yes, on a case-by-case basis. Call our office for more information.

11. Does Dr. Bernabe perform surgeries or admit patients at other area hospitals (Visalia, Fresno) ?
        --No. Currently, Dr. Bernabe performs outpatient surgeries at the Kerr Center and inpatient surgeries at
           Hanford Community Hospital. In the future, he may obtain privileges at other facilities, schedule permitting.

12. Is Dr. Bernabe part of Adventist Health?
         --No. Dr. Bernabe may operate at Adventist Health facilities but he is not employed or part of Adventist Health.
            Advanced Orthopedics is an independent entity owned and operated by Dr. Bernabe.

13. I am currently under the care of another physician. How do I switch over to Dr. Bernabe?
         --Please contact our office at 589-6294 and our experienced staff will be happy to assist you.

14. Do you accept Workman's Compensation insurance?
         --Due to the enormous amount of paperwork involved with Workman Compensation injuries, we accept this
            insurance only on a case-by-case basis. Please call our office for more information.

15. Is your staff bilingual?
         --We have bilingual personnel. We are fluent in English, Spanish, Tagalog, and American Sign Language.

16. What days are available for patient appointments?
         --We are open 5 days a week but only see patients on TUESDAYS and THURSDAYS.  Dr. Bernabe operates
             on Mondays, Wednesdays, and Fridays. You can speed up your initial visit by downloading these required
             forms and filling out the information prior to your appointment: Patient InfoMedical History Page1,
             Medical History Page 2, and HIPAA Form.
